Applications

 
Please read the Program Descriptions section of this site or email info@edeninc.org  to confirm that you qualify before filling out an application. If you qualify, you may download the appropriate application and mail it to EDEN. You must submit documentation of your disability and income with the application.  

Be sure to fill out the application completely and include all necessary documentation. In addition, keep in mind that these programs have lengthy waiting lists. It can take up to five years to access a housing unit. 

If you wish to fill out an application that is not available on this site, check the Program Descriptions section. The program may be closed, or you may need a case worker from an agency to get the application from EDEN. 


  • The Liberty Application

    The Liberty is the new 72-unit supportive housing project that serves disabled, homeless individuals. The building is located at 10004 St. Clair Avenue in Cleveland and will open in January 2008. To qualify, applicants must meet all of the following criteria:

    • Applicants must be homeless and meet HUD’s definition of chronic homelessness. This means the applicant is currently living in an emergency shelter, or on the streets and have been continuously homeless for one year or more or had at least 4 episodes of homelessness in the past 3 years


    • Applications must have a documented disability. Priority is given to applicants with mental illness, drug/alcohol dependencies, or HIV/AIDS.

    EDEN will verify all disability and homelessness information and will conduct assessments to determine whether chosen applicants qualify. The maximum occupancy for each efficiency unit is one person. In addition, all households must meet Federal Low Income Housing Tax Credit Income Guidelines. Full-time students are not eligible for this program.

    Applications will be considered in the order in which they are received. Applications will be reviewed for eligibility and applicants will be asked to participate in at least two interviews. At the time of interviews, property management staff will review the client's financial, credit, criminal, housing, and employment histories. Acceptance for The Liberty is based on all these criteria. At no time during the application process will clients be guaranteed apartments until they sign a lease.

    Applications must be hand delivered or mailed to EDEN’s offices. Faxed applications will not be accepted.

    • Emerald Commons Application

      Emerald Commons is the new 52-unit supportive housing project that serves disabled, homeless individuals. The building is located at 1976 West 79th Street in Cleveland and opened in December 2006. The program currently has a waiting list. To qualify, applicants must meet all of the following criteria:

      • Applicants must be homeless and meet HUD’s definition of homelessness. This means the applicant is currently living in an emergency shelter, on the streets, or in transitional housing after coming from a shelter or the streets.


      • Applications must have a documented disability. Priority is given to applicants with mental health illnesses, chemical dependencies, or HIV/AIDS.

      EDEN will verify all disability and homelessness information and will conduct assessments to determine whether chosen applicants qualify. The maximum occupancy for each efficiency unit is one person. In addition, all households must meet Federal Low Income Housing Tax Credit Income Guidelines.       • EDEN-Owned Housing Application
        To qualify for this program, a household member must have a severe mental illness. The waiting period is approximately 5 years long.
       
      • HAP Application
        This application is also used for several other EDEN programs, but you may only submit it for HAP. A household member must have a documented mental illness to qualify. The waiting period is approximately 5 years long.

      • Wieder-Silver Manor Application
        Applicants must have a severe mental illness to apply. This program matches two mental health consumers and places them in a two bedroom apartment. The waiting period is approximately one to five years.
       
       
      • Third Project Apartments
        This HUD-subsidized building has 7 units, and each unit is shared by either two or three roommates. Applicants must have a severe mental illness to apply. The tenant's portion of rent and utilities is based on percentage of income. There is a waiting list for this program. Eligible applicants are strongly encouraged to apply through their mental health agencies.
