Program Name: Housing Choice Voucher Program (HCVP) – Mainstream
Type of Housing: Permanent housing subsidy with private landlords: singles, duplexes, multi-family, and apartment buildings.
Locations:  Scattered sites throughout Cuyahoga County (Cleveland), Ohio.
Eligibility Criteria:
  • Must meet Federal very low-income limits.
  • Must be currently on a waiting list, which is a closed system.
Required Documents: Completed application and supporting documents, which include a background check, verification of all household income, copies of valid photo ID’s, birth certificates and social security cards for all household members.
Background Check? Yes
Tenant Rent: Household pays 30% of their gross adjusted monthly income towards rent & utilities.
Applications: No; the wait list is closed.

All applicants must submit the required Disability Verification Form.

Demographic information is collected for all applicants via the HMIS form.

Income information is collected for all applicants via the Income Self Certification Form.
